You should complete forms only as directed by the Azimuth staff.

With the exception of the "Microsoft Teams - Directions for Clients" (intended to be read online), the remaining forms are available for downloading and completing prior to your first visit.
Do not fill our the forms using a browser. A PDF Reader is required to fill out the forms. If you need a reader you can download a free one at https://get.adobe.com/reader/
Once you have completed the forms, email to Azimuth at least two days before your first counseling session.
